How it is calculated
Annual budget = employees × policy per employee + family members covered × policy per family member — leave those two fields at zero if there's no family coverage.
Policy cost varies a lot by coverage scope (outpatient care, dental, hospitalization, emergency care abroad) and the insured group's age mix — get the current rate from the insurer for the exact package you want rather than relying on the default value.
Insurers typically discount for volume with a large insured group — if you already have a quote, use its rate, which can be noticeably below the insurer's base price list.
